Kidney transplant is a treatment, not a cure, and therefore the best diet post-transplant is similar to that in early stages of CKD to keep the body and the transplanted kidney healthy. 

Polycystic kidney disease (PKD)  progresses quickly to kidney failure. There are some dietary modifications that can help somewhat to slow progression of the CKD.

Diet has a very important role in preventing the recurrence of kidney stones, which is very common.
